CHN is pleased to offer Ophthalmology services at our Berlin Hospital campus. These services are provided by Consulting Physicians with scheduled office hours at our Hospital.

The Clinic is located in the Berlin Family Eye Care building (just to the East of the hospital), 269 Memorial Dr, Ste 102.

To schedule an appointment, please contact your Primary Care Physician for a referral or contact one of our providers (listed below) directly.
