Healthcare was created because of community need -- and Community Health Network was created for the same reason. Partnerships among providers and the support of area residents who give their time, dollars and talents allow CHN to fulfill our mission of improving the health and quality of life in the communities we serve.

Beyond hands-on medical care our CHN Foundation, a 501 (c) (3) charitable organization, is available to help area children, adults, and families through our Lucille Schultz Caring for Kids and Sandy Tincher Trust Funds. 

Lucille Schultz Caring For Kids, supported by funds raised through the annual CHN Charity Golf Classic, helps children and their families who experiencing catastrophic medical needs. The Sandy Tincher Trust Fund, established in memory of local resident Sandy Tincher, provides financial assistance and supportive services to adults experiencing serious medical problems.

Application Process

Click here to apply for financial assistance through an individual grant from the Lucille Schultz Caring For Kids Fund or Sandy Tincher Trust Fund. If you would like an application mailed to your home, contact the CHN Foundation office at (920) 361-5734 or send an email to kjuday@partnershealth.org.

After you have completed and returned your application, the CHN Foundation Grants Committee will review your request and contact you with their decision within 4-8 weeks.

Donation Information 

The CHN Foundation relies on the generosity of area residents who provide the financial resources to reach out to their neighbors with dire health needs.

All or a portion of your contribution is tax-deductible as allowed by law.

Our team of volunteers includes more than 220 area residents that donate nearly 900 hours monthly -- providing critical support to CHN staff, patients and their families.
