Transitional Rehabilitation

“Providing Tools for Recovery”

Due to Medicare and other insurance reimbursement rules, many hospitals are required to discharge patients as soon as they are stable and are no longer in need of acute care services. Patients and their families are then faced with the decision of whether their loved one may be in need of further inpatient skilled services. If you have had a hospitalization, your Medicare coverage may follow you to Juliette Manor for short term transitional rehabilitation services.

Juliette Manor’s transitional rehab provides continued inpatient intensive rehabilitation and skilled nursing services. This short term rehabilitative stay can further prepare someone for a safe and successful discharge. By choosing to come to Juliette Manor for extended rehabilitation services, you will become the leader of a large team dedicated to providing you with optimal care and services.

While admitted you will have access to:

  • Therapy services including: Physical, Occupational, and Speech therapies
  • Billing and Insurance Assistance
  • Assistance with completing advance directives
  • Full service discharge planning including: ordering needed equipment and referrals to service agencies as needed.
