Our History
When community members pooled their resources to form the Berlin Hospital Association in 1911, little did they know how much their hometown hospital would impact the local quality of life the next 90 years and beyond.
The Berlin Hospital Association - or Community Health Network as we know it today - has evolved from one small hospital, serving the Berlin community, to include two hospitals, a nursing home, nearly two dozen outlying community clinics, a retail pharmacy and a rehabilitation program serving nine surrounding communities.
Today, CHN works with community members to meet the needs of families in the east central Wisconsin area through a dynamic collage of services. And we've developed strong, streamlined relationships with outside providers to ensure you receive the highest level of care you need.
