Continue the tradition of community healthcare

as a member of Community Health Network

 

(BERLIN) – Community Health Network’s (CHN) 2003 membership enrollment period is now in progress underway -- and is open through November 15th. 

 

For the same $2 fee that was assessed in 1911, CHN Membership is open to all residents, ages 18 and older, of the organization’s service area (which is defined as Green Lake, Marquette, Waushara, and portions of Winnebago and Fond du Lac counties).

 

More than 90 years ago, when community leaders proposed to build a local hospital, community supporters came together and donated $2 each to form the Berlin Hospital Association. That continued support has followed the organization's progression to CHN and the diversified family of services offered today.

 

Community Health Network is a continuum of healthcare services which also includes Berlin Memorial Hospital, Juliette Manor, Juliette Terrace, Partners Family Pharmacy, CHN Rehabilitation and Occupational Health, and the CHN Medical Group, consisting of more than 110 multi-specialty providers serving nearly 20 community clinics. Community Health Network is further affiliated with Wild Rose Community Memorial Hospital.

 

By joining CHN, members vote on Board of Directors candidates, provide input to organizational leaders, and are invited to attend CHN’s Annual Meeting held scheduled for January 13th, 2004. The Board of Directors features a dedicated team of local community leaders, who share their business management expertise, and includes: Steve Sorenson, Ripon; Jodi Timler, Green Lake; Hank Derleth, Berlin; Mike Chier, Berlin; Terry Kohlman, Princeton, Fred Gellerup, Wautoma; Dr. Roger Kjentvet, Wild Rose; Jim Roberts, Green Lake; Dr. Mary Leikness, Wautoma; Richard Reese, Wautoma and James Clark, Markesan.

 

Through the leadership of the Board of Directors, CHN has enjoyed steady growth despite being situated in an industry largely regulated by governmental agencies. As Green Lake county's largest employer, CHN employs more than 800 associates and has 22 locations serving 10 communities throughout a five county region.

 

Those interested in enrolling as a CHN Member must complete an official membership enrollment form, available at any CHN community clinic or the CHN Corporate Services office, located in Berlin Memorial Hospital at 225 Memorial Drive in Berlin. Enrollment forms must be received no later than 5 p.m. CST on November 15th, 2003.
