Join CHN in the spirit of giving this holiday season

 

 

(BERLIN) –The spirit of giving is in the air at Community Health Network (CHN) this holiday season.

 

The annual Keep Me Warm Tree project at Berlin Memorial Hospital (BMH) and the CHN Foundation’s first Gift of Health annual appeal are underway – creating opportunities for CHN physicians, employees and volunteers, along with the general public, to help those less fortunate in the east central Wisconsin area.

 

Keep Me Warm Tree

The "Keep Me Warm" clothing program provides sweatshirts – from newborn to adult sizes – to families and individuals to help them “keep warm” through the winter months. Collected items are distributed to local families, senior citizen centers and social service agencies throughout Green Lake, Waushara and Marquette Counties.

 

Those interested in donating can simply visit the “Keep Me Warm Tree” displayed in the lobby at BMH – located at 225 Memorial Drive – and remove an “ornament” from the tree. Then, the donor can purchase a sweatshirt that represents the anonymous individual’s size and gender listed on the ornament, and return unwrapped sweatshirts to the “Keep Me Warm Tree.”  These items may be dropped off or sent to BMH “Keep Me Warm Tree:” 225 Memorial Drive; Berlin, WI 54923 by December 15th.

 

The Gift of Health

The CHN Foundation has launched its first Annual Appeal to raise monies for local children and adults with serious medical needs – and promote the health and wellbeing of people in the communities it serves. The Annual Appeal consists of various giving circles:

 

Bronze Member– Up to $249

At the Bronze level, donations may be used to help fund prescription co-payment costs.

Silver Member – $250-999

By participating in the Silver level, gifts may be used to help fund lodging and transportation costs for families who must travel for specialized healthcare.

 

Gold Member – $1,000-2,499

Donations at the Gold level may help pay for deductibles and co-insurance for families requiring intensive medical care, such as surgery, transplants or chemotherapy.

 

Platinum Member – $2,500 and over

Platinum donors may help families obtain specialized medical equipment, such as a power wheelchair, that is medically necessary but not covered by health insurance.

 

Because it is a 501 (c) (3) charitable giving organization, contributions made to the CHN Foundation are tax-deductible as allowed by law. Anyone wishing to give The Gift of Health may forward a donation to the CHN Foundation; 225 Memorial Drive; Berlin, WI 54923.

 

Community Health Network is a continuum of healthcare services which includes Berlin Memorial Hospital, Juliette Manor, Juliette Terrace, Partners Family Pharmacy, CHN Rehabilitation and Occupational Health, and the CHN Medical Group, consisting of more than 110 multi-specialty providers serving nearly 20 community clinics.
